Get services
Once you can get services from us, you get a service coordinator. They help you make a plan and get the services in it. Most services are free, at any age and any income.
Already with us?
Your service coordinator is your main contact. Call or email them about any service.
Your service coordinator
Your service coordinator helps you understand the rules and how things work. They help you speak up for yourself or your family member. They help you find and get services.
Behind them is a team: clinical staff, intake, finance and more, all working to get the right services to you.
Your plan
You ask for services through your plan. For people 3 and older, the plan is called an Individual Program Plan (IPP). For babies and toddlers, it is an Individualized Family Service Plan (IFSP). You and your planning team write it together.
